Before Your Appointment

These steps are optional but highly recommended. Completing them means Dr Taiwo can spend your consultation on clinical insight and your personalised plan — not collecting background information.
Complete your clinical intake form
Tell us about your medical history, current medications, symptoms, and health goals. This takes approximately 10 minutes and ensures Dr Taiwo can focus your consultation on clinical insight — not background admin.
Upload recent NHS blood results (if available)
If you have had blood tests in the last 12 months, please upload them. You can download your NHS results via the NHS App, GP Patient Access, or Patients Know Best. Accepted formats: PDF, JPG, PNG.
Sync your wearable data (optional)
If you use an Oura Ring, Whoop, Garmin, or Apple Health, you can securely share your recent sleep, HRV, and activity data. This gives Dr Taiwo objective recovery and structural data before your appointment.
